Forms you need to submit

Membership Form

The membership form is now available online for you to submit your membership for the 2016 season.  This form is accessed via this website i.e. "Membership 2016" tab.

Please do not pay your membership fee until you receive an invoice from the Financial Office.  If you don't receive the invoice within 2 weeks of submitting a membership form please ensure to email the Finance Officer advising that you haven't received an invoice.

Availability Forms

The members are required to complete and return the Availability Forms to the Appointments Officer by the prescribed dates on the forms.  This will enable the Appointments Officer to appoint you to matches.  These forms are now available in the "Documents" tab on this website.  From the dropdown list select "Members Only Documents".  The forms are in the "Forms" tab.  

QCSRA Banking Form

Please complete the banking form and return to the Accounts Officer.  This form is located at the same place as the Availability Form (as stated above).
